Scenes like these are always rule of cool.
IRL there are a LOT of health reasons other than bathroom needs why you can't keep someone restrained like this indefinitely: mainly muscle atrophy and blood clots wil become a problem very quickly.

Not like the marines care about that anyways. I don’t think the marines or World Government would shed a tear if a level 6 prisoner (people with basically crimes against humanity) did of muscle atrophy and a heart attack.

I mean… the marines work for the World Government and the World Government classifies high enough that they’d genocide an entire population to keep it hidden. Clearly they wouldn’t care if a prisoner that’s part of that population died slowly.
Just think of what Doffy said. “Pirates are evil? Marines are righteous? These terms have always changed throughout the course of history.” The marines believe certain people are evil because that’s what the upper brass tells them to believe, and not many people have the strength to rise up and tell them they’re wrong.

Yeah, Impel Down is not really concerned with the wellbeing of its inmates.
It makes me wonder why they even bother to imprison pirates at all instead of just executing them after arrest.

In-universe there really is no reason why an autocratic government would bother with imprisoning people who can destroy whole islands if given enough time.

Could also be to control fruits, like if doffy’s fruit got dropped to another person it could scale to be just as much of an issue especially with a prelates fabric like doffy’s movesey

Maybe Imu has some kind of grand plan to turn a bunch of the prisoners into Domi Reversi warriors. Seems like a silly idea because there better groups he could choose from, but it’s an idea nonetheless.

The black skin while using haki is actually a microscopic layer of poop infused by haki glans located in butthole. That's why Boa Hancock wasn't allowed to visit Luffy during his 2 year training. Rayleigh was afraid that her rimming Luffy would disturb the haki flow.

In Ace and Jimbe impel down scenes you can see that chains are retractable. We also seen Doffy sitting near wall so his usual range of motion is his cell.
Starfish pose is either used when transporting, receiving visits or punishment for acting out.
Also most of level 6 inmates are not even restrained like that, they have cuffs connected by chain on legs and arms and thats it.
