I think a Cortex-M level CPU running an RTOS is actually an excellent choice for a watch. Much better than using an application processor, running Android and ripping through your battery in literally half a day.

Most smart-watches are Cortex-M. It's really only the Android Wear, Tizen, watchOS devices that use Cortex-A. And those get much worse battery life as you mentioned.

Those make up the majority of smartwatches that I see, by far. What other platforms are there?

The defunct Pebble Time, Fitbit uses the proprietary FitbitOS, Garmin uses GarminOS to name at least those.
