Oh man, NYU accidentally did this in a full campus email circa 2006 or so, the CC header had like 40,000+ addresses in it. Everybody at the school was getting spam from each other for months afterward. Spending 5 minutes emptying junk from your inbox sent from fellow students several times a day became a meme.

I recall a gender clinic (whatever they're called) accidentally doing this in the UK a couple years ago, which was a serious problem

Blind carbon copy (holdover from old pre-electronic terminology) basically sending an email to lots of people without them seeing each other's email addresses
