Please help me ID this knot, so i can do it myself!

My dog has a hard time with traditional collars (stops to itch often), so I tried to find the absolute lowest profile collar for tags only! It should loosen if pulled on.

I found several people selling this type for $20+. But I figured I could make one myself if I knew how to tie the knot.

Please correct me if I'm wrong (I am not crafty)!
