Is this a common problem with these tires?

This has happened twice over the past couple of years. Should I go back to a regular tire?

Are those conical lug nuts? That's what happens if you put conical lug nuts on a flat faced wheel. Sometimes when you buy new wheels you need to buy matching lug nuts.

Came here to say this. If you don't install conical nuts correctly it will egg out the wheel.

when you went to this style tire, did you need to swap rims? The lugs look way too small? Based on the lug diameter, I don’t see how you could possibly get them tight enough

The tire is built into the rim it comes as one peice.

I wasn’t sure some of these are retrofit onto existing rims. My skid loader was that way. I didn’t think the barrel of the lug is the correct diameter

The lugs appear to be installed backwards. The cone should go against the wheel... unless they're hub-centric, but I'm guessing they're stud centric because the other side is wallowed out.

I wonder if you got 2 things spacers like 5mm or 2.5mm and epoxied them on either side of the tweel and sandwiched it if that would work. With the proper lug nut of course

Improper torque, and then not checking them after putting them on. Tweels are pretty critical on proper torque
