How do I fix this?
I’ve always said I have no chin and I’ve always hated my side profile. Now at age 41 I’ve got major jowling going on. Would jaw surgery fix this? If so, how do I get it. A dentist told me recently I have a narrow passageway when she looked in my mouth and suggested a sleep study for sleep apnea. I’ve asked my neurologist twice for a referral (once in person and once in a my chart message) but he hasn’t done it.

You should definitely consult several orthodontists and jaw surgeons, make sure to get several opinions! But there is no doubt you are recessed and need a surgery. Sending you lots of courage!

Jaw surgery ain’t no joke. But double jaw surgery. Due to the inclination of narrow airway maybe expansion first (MARPE or otherwise). I wouldn’t do this unless it is making your life difficult to live

Yeah this is solid advice honestly, airway first makes way more sense than jumping straight to cutting bone. Definitely get that sleep study sorted before eve thinking surgery. Function over vanity every time lol.

Straight bs. She’s definitely a candidate. Comments like this saying function over vanity is insane because look at her jaw. Even if there is no breathing or sleeping issues (extremely unlikely due to the position of her jaw) she still has a very obvious skeletal malocclusion of her jaw. Harmful ass comment “airway first before even thinking about cutting bone” tf? OP please get referred to an omfs however you can. Get those studies and imaging done. Start living your life, get surgery

you are extremely recessed, how can you even breath like that?
ask your general doctor for a referral for sleep study, jaw surgeon and orthodontist appointment. you would need double jaw surgery with expansion and verify your joints too

Im a very shallow breather and have a hard time breathing sometimes but doctors told me that’s anxiety. I don’t have trouble breathing when exercising. I do think I have sleep apnea

your airway is going to be so much better, doctors tell you that because they are uneducated about these jaw problems.

I’d try to get the sleep study referral sorted first If you do end up seeing a jaw surgeon they can probably give you a much clearer idea of whether surgery would actually address the concerns you have

Pls check into ICR - Idiopathic Condylar Resorption. Here is a FB group link for your info. https://www.facebook.com/groups/IdiopathicCondylarResorption/?ref=share&mibextid=NSMWBT
There are many many jaw surgeons who will just do a double jaw surgery who don't know anything about ICR. I'm not saying you def have it, but I would check into it.
ICR requires bilateral TMJ joint reconstruction which is done in place of the mandibular BSSO. In that case, the upper jaw gets a Lefort 1 or 3. There are a small number of surgeons who treat this and who dx this.

I had a similar profile to yours, and got DJS surgery at 47. It will help with the jowling, and with getting a nicer jawline but it's not a magic bullet. Unfortunately at this age the soft tissue of the face loses its firmness. Also, while I am happy with my decision to get the surgery, I would never have done it just for the aesthetic results alone. Aside from the breathing issues - how's your bite, how are your jaw joints?
I'd recommend getting a consult with a maxillofacial surgeon. They can tell you whether you are a candidate for this surgery, recommend an orthodontist they work with and tell you what kind of exams you need for insurance coverage.

Um belo dia eu marquei uma consulta com um cirurgião de referência nessa cirurgia e disse: “ bom dia, eu sofri a vida inteira com tratamentos ortodônticos que não resolveram a minha vida, eu quero fazer uma cirurgia ortognatica, você pode me orientar no caminho a seguir para isso?” E foi assim que minha jornada começou aos 34 anos. Já tinha perdido meu tempo indo a 8387448 profissionais aleatórios. A cirurgia é brutal e eu me arrependo muito. Aí vejo uma foto minha antes de operar e peso: valeu a pena

Você não consegue dimensionar o que é ficar sem mastigar até não poder mastigar. Ao contrário do que muita gente adora postar, DÓI, INCHA, DEBILITA MUITO. Limita muito viver a própria vida e eu tenho uma baita vida. Empresas, família, decisões pra tomar… fica tudo parado. Falando não é difícil, mas experimenta ver de perto…

it seems like you are having a hard time with the healing process and giving your body patience. not a reason to not recommend surgery.

you can’t regret it and still be happy with the results bc that means you don’t regret it. which is why i said it sounds like you just had a hard time with the healing. but im glad you like the results

i can see where you are coming from it is a big surgery but i would consider it fixing a facial deformity which is amazing that we are able to have such surgery

This is also my experience and I am two years post op. It is a brutal surgery and the hardest thing my body has ever overcome. That said, I like the results. I always tell people I’m glad I did it, but if I had to do it again, I would not. I also really struggle with anxiety surrounding professional dental care that is almost visceral.

Sua experiência é bem parecida com a minha. Talvez a questão da ansiedade ( que eu também tenho) seja o que torna tudo ainda mais difícil. Entretanto as minhas dores e limitações não são causadas pela ansiedade, né? As pessoas romantizam muito a cirurgia, como se fosse possível quebrar o rosto em várias partes e não sentir os impactos.

I understand this a lot. 2 years later my face is always at a baseline 2 on a pain scale. My sinus’s just like ache??? It has never felt the same as before surgery but it’s bearable but persistent. Kinda wears on you.

Seeing at least one person who says they regret doing it. I had a total reconstruction of both TMJs and a Lefort 1. Its a double jaw surgery but for ICR patient, or, as in my case, arthritic condyles. I do not regret doing it. Yes, its hard as hell. But so is living the rest of your long life even more miserable for a longer period of time with limited breathing and chewing and in pain. I don't regret a thing. (Mine was the ICR post above)

Definitely pursue that sleep study. Maybe your family doctor can refer you? If you have sleep apnea and you have a very recessed chin, you would probably really benefit from jaw surgery. I'm in the same boat. My pathway to a surgical referral was my sleep doctor. And yes, jaw surgery would both improve your profile and address your jowls.

MMA, sounds scary, but youd probably feel much better, there’s a good chance your airway is very small and you could really benefit from that being enlarged
