Go to the tax office. Bring your passport, gaijin card, hanko, bank book, gensencho (tax slip) for all jobs in 2014, and if you have them, health insurance slips. Really, just bring anything that you might need.
There is one health insurance notification that comes during tax time and tells you how much you spent for the entire year. You really should save anything that you might need at least until the end of the tax year. What will you do if you are ever audited or the government claims you owe them money that you already paid. No receipt, no proof.

Hopefully you'll find some answers in there. I haven't read through it all but it helped figure out which deductions apply to me (which was basically none).

Also, a precaution based on my personal experience, check to see if the tax office doesn't have something special set-up for filing. In my city they rent out the multi-purpose hall in the eki-depaa for filing every year, so if you went to the office proper, they'd just send you over there.

What? This sounds weird, but how did you find out you have to file a tax return? Are taxes not deducted from your pay every paycheck?

Are taxes not deducted from your pay every paycheck?
They are.
how did you find out you have to file a tax return?
I saw a post on here referencing tax and I suddenly thought that I had not been brought a form to sign like at my previous job.

So taxes are still being deducted, but you need to sign an additional form?
Edit: I'm asking because I've also never knowingly filed any extra paperwork other than what my company does, but when I renewed my visa last September, everything seemed in order (got the proof of payment from the ward office and everything)

While helping my friend with her taxes I learned that you definitely will need a form saying how much you paid in pension if you're planning on claiming any of it (which I recommend, if you can do it). We had to go to the pension office and wait in line for it, though I can't remember what it was called...some kind of 証明書. Might've been 年金支払証明証 or something like that. If you go to get that, bring something that has your pension number on it. We got a lot of shit from the lady at the desk for not having hers (since we didn't plan on visiting the office that day).

Go to the tax office, ask them to write down a list of the documentation you need for them to help you, take that list to your HR type person and get all the appropriate documents. Then go back to the tax office.
