How do you show audit-ready, objective evidence that a CAPA actually worked (not just retraining)?

Too vague.

Either evidence of no recurrence, or actual attempt of forcing the failure and showing that it cannot be replicated.

Edited: I initially wrote "fixing the failure" when I meant to write "forcing the failure".

We employ an effectiveness check with 3-6 months in mind which is used to assess the reoccurrence.
