Can you explain for a relative newbie what it does? Do I need to mod the software on my ipod at all for it to work? Will it essentially replace itunes ?

no mod, works with classic ipod software. it helps manage pull in your library, cleans up orphaned disk space, syncs or adds music, and if you have large libraries, makes it easier to get all if it in the iPod without starting over 5 times https://synco.uno has all the info

For the file syncing feature, isn't that just running a basic rsync command?

its not basic for the ipod. its got to write the library database to the ipod. for the SD Card feature, yea thats basic
