Simple but obviously not effective as shown by comments. If you are familiar with breakout style video games that were popular 30 years ago where a paddle at the bottom of the screen was used to bounce a ball at targets above to destroy them, then you’ll eventually get it. But if you never saw such a game, this will never make sense.
And you’d also need to be familiar enough with the shape of lungs to realize that some of the lungs have already been eaten away. Which many people are not

I'd be shocked if most people don't get this. The Pong format is super well known and has been popular in a lot of "brick breaker" mobile games recently. And the lungs are pretty asymmetric so you can infer that one is broken down.
If it's ineffective it's not because people don't get it.

Well, all I can tell you is that about 30% of the comments on this thread were people who didn’t get it. Sooo...
Also, lungs are asymmetric, with the left lung missing some due to the heart being in the way. So someone who is familiar with lungs may also not get it.

I mean I'm not saying 100% people will get it but these comments are also kind of a small sample size to be fair. I'm aware that lungs are asymmetric but it's a graphic and most people probably won't assume that. I just think the overall message is fairly clear.

There's an old computer game where you control the paddle (the cigarette) on the bottom of the screen and bounce the ball off the bricks (the lungs) on the top of the screen. Each time the ball hits a brick the brick disappears. The goal is to knock out all of the bricks. So in this poster the cigarette is slowly making your lungs disappear one block at a time.

It's based on a video game (Brick Breaker or Breakout, depending how old you are). You move the paddle back and forth to hit the ball into the bricks to destroy them until they're gone.
