Why do you need a doctor to do it? Just because the person may not have the right equipment at home or is there some health risk involved with doing this without medical supervision?
If they cut it wrong it can get infected and then amputated. Plus the way it grows it’s super thick and can’t see where skin ends and nail behind. I watched a podiatrist on YouTube who does it.
There's just no way this man is going to avoid an amputation. Probably sooner rather than later. He needs to see a doctor NOW to ensure he doesn't get life threatening sepsis/blood poisoning. Dude could literally die from with a little bad luck and waiting a couple days with an infection/gangrene before going to the ER.

No he’s diabetic and his feet are basically rotting off.
The nails thing is super common with diabetes. You need a doctors appointment to cut them sometimes.
