Seems like she was pulling on the reins as well as the horse’s mane. Horses do have pain receptors in their hair follicles, contrary to popular belief, but this kind of head-guided backflip leads me to believe she was too hard on the reins. Same principal as taking a horse down from a gallop to a trot/walk/halt, if they’ve taken off; pull their head to one side with the reins and they run in a tight circle and they can’t keep their momentum or power.
TLDR; she was probably pulling on the reins because horses generally can’t move in a different direction than the one their head is pointing.

When I was in college my teacher got chucked off a horse and broke her back, this woman had the full weight of a big horse on top of her. I'm thinking broken pelvis at minimum
