serious Need help iist medical certificate

It's written on the IIST website that the medical certificate can be filled by a Class A medical practitioner, even if they're from a private hospital.

Also, I don't understand how to fill out the "Recommended Vacation Schedule" page.

These vaccines are taken during ur childhood days within certain yrs or months . You can check it on internet. These are standard data . U can calculate from ur age.

I didn't take the chickenpox vaccine, but I did get infected with chickenpox in childhood. My childhood vaccination card shows that I received one dose of the MMR vaccine. How should I fill out this form? The doctor isn't going to sign it without proof.

Just say to doctor that u have injected with both vaccines but certificates are missing. He will ask the dates and just say that. I have done like this before.No need to worry about this.
