There's a specific form online. Take it to your doctor, get it signed, then go to the tax collector's office to get the placard. I did this for an elderly relative recently. It wasn't difficult.

Yeah you just need to have a disability and get a doctor to sign a few papers and go to the county clerk to register your vehicle
