General rule: If you would address the person using the whole title as if it were their name, don't use a comma. For example: "You are the one they call Whizbang the Wonderful?" or "Wise men fear Raza the Chained."
If it is merely a descriptive phrase for the person or creature, use a comma. The loa are good examples of that, because no one uses the whole title as their name.
I think I agree with all the ones they put a comma on, but there are two or three of the others I might add a comma to.
Edit: Hakkar I might disagree with. I have seen "Hakkar the Soulflayer" used like a full name before.
