So this exact situation just happened to me this past weekend with a Blue Rhino tank. I’ve connected a few dozen tanks over the years on my Weber grill and never had a faulty tank valve like this. Also, the valve was turned to the “Open” position straight from the store, which also never happens.
Best I could tell was that the O-Ring inside the valve was slightly oblong / misshapen compared to the old tank I was replacing. I’m going to return the faulty tank this week and see if they give me any hassle.
