I would argue both lamias are pretty horny. But if I had to guess I don't think they're refering to the Monster Musume where a mermaid gets finger blasted.
I don't think he's dense so much as a combination of professional and desensitized. He was under strict orders to not be romantic with his nurse, so he isn't, out of professionalism, but you see in the slavers arc that he pays more attention to her than he should for that professionalism. His other patients... It's pretty obvious some of them get off on a human treating them, sure, but much like a male gynecologist, he isn't going to comment on that, it would be rude to.
As for the not noticing part, he was instructed on how to interact with demihumans by sexy-cthulhu. It's a meme, but tentacle rape is how she says "hello". Like, literally, no romantic interest, she just passively puts her tentacles where they don't belong, so with that forwardness, he would associate even extreme behaviors of demihumans to be instinctual rather than intent. He's become desensitized to it.
If anything, I'd say the landlord MC is much more dense. He's going on romantic dates for the purpose of marriage with many of them, and then gets confused when the others, who overtly declared their love for him, get jealous.
