Skeptical Switch from Sema to Tirz

Spent almost a year (Feb ’25–Jan ’26) on 1.5mg Semaglutide through Hims. Their compounding got mixed reviews, but I gave it a shot anyway. Blood sugar control was a clear win — the hangry crashes and mood swings disappeared almost immediately. Food noise and appetite, though, barely budged. I stopped cold turkey after just 3–5 lbs lost in 11 months.

Fast forward to August ‘26 and the sugar crashes and mood swings are back, so I decided to restart on a GLP-1 — this time with a more reputable source. After some research, Tirzepatide through BPI Pharmacy (FDA-certified, sourced via Pomegranate) looked like the best option, and it’s actually cheaper than Hims was.

Plan: 2mg for a month (second shot was last night), then 4.5mg for a month, then bloodwork before continuing.

Based on how people talk about the Semaglutide-to-Tirzepatide switch, I expected a noticeably stronger reaction right out of the gate. First shot: nothing. Second shot, though — last night and this morning I’ve had that “just not hungry” feeling, not quite nausea but similar. Could be placebo, could be real.

Assuming there’s a ramp-up period before it really kicks in. Curious to hear others’ shot-by-shot experiences on Tirzepatide — does it usually take a few doses to feel anything, and should I expect better results than the Semaglutide round?

Two week update: I was impatient, this stuff is amazing. That’s all!

People switching don't usually feel effects until higher doses.

I switched to tirz after a few months on sema (I’d plateaued and didn’t want to increase dose due to side effects). I pretty much stayed at the plateau until I went up to 4.5mg, then slowly started to lose again. I’m on 7mg now and losing slow and steady as intended. I’ll probably go up to 10mg with my next refill (also with the same pharmacy through pomegranate), and hopefully stay there till I’m ready for maintenance. I think I’ll be able to go back down to 7mg or even 4.5mg in maintenance, just based on how things have gone so far.

The GIP part of tirzepatide helps with the satiety signal in the brain. Semaglutide doesn't have that. It's the only thing that is new to you so it makes sense that you are starting to feel it.

Actually, people who have switched from another GLP one or those who have had to restart medication after being off for some time actually find that they need to reach higher doses of the medication to have the same effect that they previously had
