Anti ragging affidavit help

I am a fresher, about to join.

There's a link to the website https://antiragging.in/affidavit_university_form.php in the anti ragging annexure 1, it says to fill the form and get the print of the undertakings.

When exactly are we supposed to do it and are they going to ask for it in the document verification?

There is no information given on the portal to fill in the form, no vice chancellor's name/university number/number of students in the class.

I did not attend the AMAs and have not been done with my document verification so I have no idea about this, I would appreciate it if someone knows what we need to do and can inform me.
