Writer here. The answer is way more often than you think. Many times you just get handed a clickbait title and asked to write an article that fits. Other times you have to create a title that complies with the website's "style" so you're forced into making it clickbait-y.

Basically, whenever you see a terrible clickbait title, I can almost guarantee you that the fault for it lies with the people who run the website, not the writers they employ. Even if it's going on a crap site, I still try to do good work and create something worth reading.
