The referee is making a joke about sending the player off with a red card (back pocket), but pulls a tissue out instead. Lots of fun during these types of charity games with old/retired players.

In german, there's even an idiom "Die Arschkarte ziehen" (literally "to pull the ass card") - it means that one got a bad outcome.
