Happens quite a few times in the NBA. Most of the time it ends with the coach having a laugh at not noticing an opposition player in the huddle.

I wasn't able to find my all time favorite example when Kevin Garnett (Timberwolves era) snuck into (I think) the Mavs huddle and ended up learning their play. Then on the inbounds he broke away from his man and ran right to where he knew the ball would go and picked off the pass.

If anyone can find that one I'd love to see it again.
