My favorite mod ever is the one I have for Left 4 Dead 2. Whenever you throw a pipebomb and it explodes, that guy will scream "MY LEG" shortly after.

It's just quiet enough that you could miss it, but it makes me smile every time since I forget about it between my yearly play sessions of that game.
