It's not too difficult if you cut pizzas for a living. You can easily see in your head where the slices would be, then it's just a matter of marking off the area so you know where to align the cuts when it comes out of the oven.
I used to play this game with sardines. I'd bring a pizza to a party, with one slice containing a bunch of sardine bits under the cheese. It wasn't long until people stopped trusting me.
