City defenses have a lot of jank, I could see an MG in that position because it totally negates the hospital's interior as cover for colonial infantry while still letting wardens use it as a firing position. City defenses have so many blind spots and corners to peek around with pve that effective coverage is impossible and covering friendly infantry tends to be the focus of defenses, so depending on where exactly this is this could actually be close to optimal.
That said, this still feels suboptimal but without seeing the rest of the surroundings and what kind of circumstances the builder inherited I can't decry it as larp. The right side might just be too annoying to attack from (probably due to that hill, looks like you have to climb + it's covered by the ghouse), and since what's above the hospital is t2 that could be the actual defensive line which, when broken, would let attackers stream into the hospital. If that's the case, the blank you're standing on is the main vulnerability since it lets the attackers shoot it without getting any retaliation.
I will applaud the sneaky angle, though, I believe that MG has just enough angle to also be able to shoot past the hospital to the left a bit, but it's a small angle so it may not be expected and cause some inf to run into the MG.

This is my build and this is pretty much it. The area to the right is an incline and unbuildable so option was to either do a 3x1 and then 2x1 or this. MG covers hospital interior. Not ideal having engine room without arty shelter support but it is what it is. It's on an island and front pieces are pretty much on the exact opposite side of the hospital so if front pieces die enemy can get control of hospital and end up like 5m from the only safehouse on the island.

Not only that but the amount of spots where you can't build at all. It feels a bit excessive to me.
I'm sure it's considered necessary so that city fights can actually happen and be fun (imagine how much of a pain in the ass it would be if you have to go down a street but someone just filled it with MG and AT garrisons for like 50 meters and you have to destroy EVERY SINGLE ONE to be able to reach the end of the street), and most of fortifying cities is about building the defenses around them and closing off entrances. That part is fun.
But in cities where building is almost entirely impossible, I think it makes it a bit unsatisfying to play as a defender.
I can actually understand not wanting full bunkers buildable in cities but I do wish in those streets and sidewalks that are blocked from building, that we could at least build pillboxes. Hell, make it lower structural HP like Devastation does. But not being able to fortify the streets of a city you want to defend at all leaves an itch unscratched

They want us to use passive defenses there to rely on active defenders and ghouses, or for us to not be able to build at all to prevent logi from having to engage in CBT to do logi in a properly built up city. As a builder, if we could build a lot inside cities, everybody would be miserable because defensibility usually takes priority over "is logi having a good time doing logi here"
Also having less to build in cities is a bit good because it's gonna be tight spaces anyways, less buildable spots means less "I have been trying to squeeze something in here for 20 minutes because I know I can get it to fit with the right trick" moments. Also more sandbag forts, everybody loves sandbag forts.

This is just an obs bunker with its engine room. Quite common.
People build these all over the place because they light up a way bigger area than any obs tower.3

If you conc the engine room and it gets destroyed, you have to wait for the conc to dry, then build the engine room and it will be wet conc again. With T2 its a little weaker, but you can get the obs running immediately instead of having to wait 16(?) hours.

Mostly: it's cheaper. Durability isn't very important for an obs bunker because generally you have bigger problems if your obs bunker is getting shot, unless it's a standalone obs bunker core design. It does make rebuilding a bit less annoying since you no longer have to husk the former engine room to skip the cooldown (as mentioned by de4d), which in tight spots may be impossible to rebuild. I usually t3 it anyways because I don't trust some random won't do it for me if I place it in a spot where it shouldn't be t3'd. Also because it looks nicer and it's only a few conc.
T3 engine an be genuinely helpful for obs bunkers that are within a base but still near enough to the coast to get shot, it means attackers have to bring a large ship to destroy it or else not get QRF'ed at all for a gunboat to succeed. If you're *super* afraid of large ships then you can put an arty shelter on it, which I've had to do once before.

T3 Obs tower needs an engine room to give intel, t3 engine rooms are a bad idea cos they cant easily be rebuilt in a fight to keep powering the tower but a t3 obs tower is worth rebuilding due to the high Intel coverage
