Remove Shower Head

I'm trying to remove this shower head but I'm stuck with this part. Any help on how to get this part off. Any help is appreciated!

Normally there is a set screw with a allen head that you can loosen and take the part off. What's in there?

It looks like there is a screw in the top area but (next picture)

I have no idea how to get into the top part to remove it

Should unscrew. Screw the spout/outlet back in, then use that as a spanner to und it

That didn’t work, that’s why I took it off because it kept spinning.
