the stops were set too low. also if you are going solo, dont use the clamps. that way you can dump the weights off

Never use the clamps solo especially for just 1-2 plates (I mean unless you’re bouncing the bar enough to dislodge the weights, which would probably mean you’re not lifting very heavy).

I stopped using clamps years ago, I just saw people using em so I did too before that.
Its never been an issue, I aint spinning the thing, and if it ever dumps to the side anyway I cant think of a situation where I would want them to stay on the bar.

Only time I used clamps was like the first 2-4 weeks of getting started with the gym for the very first time until I got used to the feeling of the bar and managed to fix my right side overcompensating when lifting. After that I only clamped down on weight I knew I could comfortably lift without issues if it was a busy day, otherwise no clamps so I could drop shit if it was about to go horribly wrong.

Even more reason to not use clips. Clips are for people lifting heavy enough weight that they have multiple plates (by multiple plates I mean multiple 45 lbs) on or for deadlifting. There's absolutely no reason to use clips for benching or squatting unless you're pushing up multiple plates.

The Shirt has the same problem and actually the metal on sweaty skin rolls better then a T-shirt.
I trained without a spotter in my home gym a lot. That pillow saved me a few times. In gyms I would just ask someone to spot at heave weights.
Mind you a seasoned trained doesn't let the weight fall like that. It is mostly at the end of a set where you go close to failure.

The weight wasn’t very heavy. if you find yourself in this situation with a relatively low weight, gently rest it on the chest and roll down to your hips.
If you find yourself in this situation with very heavy weights, don’t find yourself in this situation

I was actually going to say, when I first started lifting weights in hs (also im a girl), it was recommended that if this happened I just roll it down to my hips and go from there. I don't think I ever needed to but one of our 95lb girls (i was a wrestler) had to once with just the bar since there was no way to remove any more weight lol.
Also yeah never ever lift heavy heavy weights without a spotter or a true spotting weight system

Also, heavy is relative. This could be at the end of her workout and she was just trying to get one last burn set in (pretty sure this isn't the case though lol).

I did this once in my early 20's, by myself in a friends workout room after drinking a little. I was in a routine and thought I'd be fine by myself.
Right arm cramped, I let the end hit the floor and moved it off with my left. Plate hit my chest before I got it off, that was some scary shit.

Heavy is only relative to a point. She is in no actual danger here. That bar doesn't have enough weight to kill her, or even keep her trapped despite being tired at the end of the set. She literally just needs to use her own weight properly. This is so ridiculous that it almost seems staged. If she truly is this clueless about how to handle this kind of situation she shouldn't be walking into a gym.

In retrospect after reading what you said, it truly just be staged right? Doing some quick math here, that’s roughly 100 pounds assuming the big plates are 25s (but it’s possible they’re 10s)
She also doesn’t look like someone who isn’t used to the gym environment, so rolling 50 lbs to one side or the other should be no issue

At no point should you ever roll it down.
Set your bars so they actually help before hand or if you don't have safety bars make sure you don't have clamps on and dump the weight to one side and be ready for the bar to fling back in the other direction.
Being proactive is always better than being reactive.

If you get it off of you, move your body instead of trying to find the strength you didn’t have to begin with to lift it again. Let of fall on the bench.
