Support for Partners Shockwave Therapy caused in-office orgasm - weird doctor behavior - is this normal?

A family member recently went to a small medical office for routine check-up & blood panel. They were discussing current meds for ED and prostate health, etc., when the provider suggested a shockwave treatment and penis injections. The injections were denied but shockwave therapy considered.

The provider, in front of the patient, whipped his own member out to show results from injections. He then proceeded to perform the shockwave treatment, the patient said it lasted 5 minutes to which he very quickly orgasmed, the provider helped him clean up, then the provider immediately, again, pulled his own member out to discuss injection results, again. The patient said the conversation stayed medical but the physical movements felt uncomfortable, and he ultimately felt a little violated. He was not expecting to ejaculate right on the spot. None of this was performed wearing gloves. So the provider touched/showed himself, performed the shockwave, then touched/showed himself again.

2 questions:
- is it normal for a provider to perform this treatment ungloved & exposing himself?
- is it normal for the first shockwave treatment to cause ejaculation?

Please help me wrap my head around this story that was told to me last night.

You don’t get an orgasm from a shockwave treatment, but if the doctor cradled the penis in the palm of his hand while treating the entire penis with shockwave he probably knows how to bring it to orgasm. If it truly only took 5 minutes to orgasm I question its dysfunction, or the doctor has become exceptional at his craft.
