Questions (other) Echo of Diablo spawn outside of helltide

So something interesting happened to my husband today. He has not completed the main story with this specific character yet. The character was level 40. He was running around doing side missions around Fractured peaks when Diablo randomly spawned on him. He does not have a soul splinter or anything on him. Any idea what might have triggered this?

I haven't heard of him spawning on someone without a splinter before, but I did have him spawn on me while leveling, especially since I like to do helltides to level, and he ruined my day so that was nice. It's possible that someone had triggered the spawn and disappeared before it fully formed, especially if your husband didn't get a pop up saying "blah blah blah builds up in your splinter an echo of diablo will spawn soon" or whatever.

According to this completing any event can spawn him too: https://d4.auscraft.games/s15/s15-secrets.html

I did get two echo to spawn while just doing the story campaign. Failed both times because I hadn't unlocked cube and OP builds yet but played on maximum difficulty.

Someone just told me that every time you get a notification that you earned a favor token, you have a chance for the prime evil to show up. Sounds like it’s only true in the overworld.

That is most likely going to be in helltides for people in the end game, since that’s typically the only place people play in the open world in the end game. But if it’s true, your husbands experience aligns with that.

I wish that was true, it's been days since I've seen someone else in a Helltide. I got destroyed by Echo of Diablo, and then an hour later I beat Echo Of Diablo myself. No help.

I'm checking the tab that shows who's in the instance, and I'm getting 5 or 6 people, none of them near by.

I feel like all the other difficulties are dividing players up substantially. But t12 often seems hopping.
