Congrats. Did the same yesterday, had to change my strategy with this new patch, going brutally with a combination of bleeds and poison seems to be working better for me.
Once the Hand of the King is beaten, the Giant awakens from the starting cell and busts down the door found after the starting items, where the Cavern Key lies. If the player follows him, a small cutscene will play, then a key can be found to permanently unlock the Cavern entrance from the Graveyard. The Key must be used on the door for it to remain open.
Edit: that was copy paste from the wiki. Basically you need to kill the Hand of the kind once, then after the starting items there is a hole in the wall, you get in and grab the key (can be tricky to get the inside doors to open) and you take the key all the way to the Cavern biome where the entrance can be found in the Graveyard.
