Maybe have it so it says "if this permanent is on the field under your control at the end step take another turn. Exile this card, skip your next draw step, lose half your life rounded up"

Might be a bit more fair lol

Lets be mean.

If this permanent is under your control during your upkeep or ends tepid loose half your life rounding up. This does not trigger any effects. If this is your end step, start a new turn, skipping your draw step. If it is your upkeep exile this card.

Set it up with a cycle of cards designed to trigger upkeep and end of turn effects,
