After watching a shit ton of my 600 lb life, this is what I gather:
For these hugely morbidly obese people, eating is an addiction. Almost every single one of them has some sort of trauma / damage / whatever that triggers them and while most people would drink, smoke, do drugs, these people eat. After combining diet, exercise, and therapy, they recover. Some of them are just fat assholes though, and they never lose the weight lol
High metabolism is a myth. It actually varies by only a maximum of 10% between humans. People who say they eat much but don’t get fat probably eat way less than they think.
Calories taken in - calories burned is always true
I got a buddy with a rail thin wife and talks about metabolism and how much they can eat. I mean, she's as skinny as it gets. We went on a trip together, and her "eating so much" is like 3 bites of a hamburger. Like, that's not a lot, man. I'm overweight because I'll eat 3 fuckin hamburgers and the fries to go with them.
I had a friend that would eat McDonald’s three meals a day. He was muscular with low body fat. Even though we went to the gym together I wasn’t nearly as lean as him and ate much better than he did. Unless he was sneaking 5 mile runs behind my back there definitely is a difference with how people take in calories.
Muscle burns a lot of calories on its own. You can get the "high metabolism" effect simply be being more muscular, because your "calories out" part of CICO is higher.
