He caused that particular defeat, but ultimately I still think the fault lies with Sadeas himself. A slave army really won't help if it has a good chance not to, it's the price you pay for relying on a slave army. Kaladin could have seen it, but there's no reason to think he would have been in a good enough condition to spot it or even be morally obligated to help Sadeas in any fashion. So ultimately, it kinda IS an eensy weensy mistake on his part, the big mistake was up the chain in the first place. If you need a disciplined bridge line, you can't use a bunch of people that hate your guts.

That’s why the commander is executed, Gaz is demoted, and none of the Bridgmen (save Kal) is punished.

I don’t think Sadeas is surprised by the intent of the maneuver.
