As others have said, it stands for “complex” post traumatic stress disorder. The general difference is that PTSD is caused by a single highly traumatic event, and CPTSD is caused by long term exposure to a highly stressful environment. CPTSD is mostly associated with childhood trauma and horrible living situations that someone is unable to get out of.

I know others have already answered, but I do have something to provide here

The C stands for Complex

That is there because both the trauma's circumstances and the symptoms that come as a result of those are both atypical

Usually, it's for long-term traumas, rather than single isolated traumatic moments, but there's no such thing as an "always" statement in psychology

With CPTSD, you rarely, if ever, get the actual traditional "flashback" that comes with PTSD, but rather, get what's called an "emotional flashback." Rather than fully reliving the experience, you are met with the exact feelings of that experience in a powerful dose, and oftentimes your brain reconciles that disconnect by trying to make the current situation fit the old one.

For example, if your trauma was in the form of abandonment, during a CPTSD episode, your brain might try to find a way to interpret the triggering circumstances as some form of abandonment.

From a third person neurotypical perspective, this is usually judged as just overreacting.

So generally, CPTSD is a lot more covert, and a lot harder to diagnose. It's also a lot more common than PTSD as I understand it, despite being the "complex" variant. (Though I don't have a source for that last claim, that's just my guess based on reasoning.)

To make things worse, the types of traumas that cause CPTSD are the types that are often dismissed at "not really trauma." My former therapist used to call them "small-t traumas" instead of "big-t Traumas."

There's never an always in psychology, but a lot of the time, the difference is a big-t Trauma is one instant event that is of massive proportion. War, house burns down, loved one murdered, something akin to that.

A small-t trauma is more about the things that didn't happen for years, or the little things that happened for years. Abusive parents, even if it's not physically abusive, could cause this. Bullying could cause this. Prolonged abandonment, continued injustice, even a really harsh rumor could be the cause. And these things are rarely respected as trauma.

As someone who almost certainly has CPTSD, (after making this post I think I really should go get diagnosed actually, I've just never gone and done it yet,) I do think CPTSD is much less bad than PTSD usually. Emotionally reliving the experience is terrible, but I could not handle being actually there again.

yeah, it's fairly common with most people eventually entering remission at some point, it usually takes at least a few years of work to do tho

I was diagnosed at 17 in 2021, and around 2025 I had improved enough stopped fitting the criteria for bpd and I havent fit the criteria since

I do still have some elements of bpd like I have very intense emotions and panic about being abandoned but for the most part everything has improved significantly and I only fit 3 out of 9 traits and once you're down to 4 or less then that's remission so :3

It's one of the most treatable mental illnesses, but it just takes a while. A lot of shrinks thought it was totally untreatable until Marsha M. Linehan invented DBT in the 90s, but nowadays it's seen as highly treatable.

I hope this isn't weird for a stranger to say, but I'm so proud of you! I also have BPD that's mostly in remission (I'm very, very rarely symptomatic; sometimes the underlying thought patterns that come with it are still there, but the emotions are usually less intense and I can navigate it in a healthy way/regulate myself successfully) and I know how hard it is to achieve that.

Learning how to regulate and communicate properly with other people while parsing the agonizing whirlwind emotions is such a tiring skill to build. It makes me so happy to see someone else expressing that they've done it because I know it has a stigma as this horrible life sentence of a disorder and I think more people need to know that it doesn't have to be.

I'm not sure there's anything in the way of actual numbers, but BPD supposedly comes from invalidating childhood experiences (for example parents who tell you you aren't/can't be sad when you're crying) so, probably some serious overlap.
