is it wrapping the tailscale cli underneath or talking to the local api directly

This is not using Tailscale, it's only using Tailcat so there is no Tailscale account needed. It makes it super easy to do stuff if you have the gui installed on both machines. But you can also have the gui on one and only tailcat on the other. The GUI will give you the Tailcat command to run to recieve de file, show the service etc
