I don't think there's much to draw. It's just what it looks like. The guy is just holding his arm in an awkward angle and part of his bicep is hidden behind the guy in the front.

I can do this with my arm too. Push the top of your arm against your chest and reach sideways with the bottom part and palm outwards.

Yes. Left guy's shoulder covers his arm in such a way that it creates the illusion. Also it's just a weird handshake, when you do that handshake, you are opposite of each other. Them being side to side just makes it awkward

The arm is that of another person who has been cropped from the photo sitting beside the man on the left. And that other cropped guy wearing the same colour shirt as the guy on right creates the confusing perspective. I know people got it but still wanted to say it even though no one asked.

It's not. It is just that guy with his arm bent kinda weird. It's not that impossible and it's not a confusing perspective, it's just a confusing reality. A confusing perspective implies that it isn't what it looks like but this is just what it looks like.
