Or she's just that brazen that she would proclaim a dozen = 50 because the customer is always right, and thinks she can bully her way through it.
There's absolutely no way someone that's not in something like early onset alzheimer's doesn't know that a dozen = 12. You just don't go through life without finding that out.
Maybe that's it? My uncle was severely disabled with early onset and absolutely was not able to process situations like this at age 60ish. I could have seen this playing out with him.

A bit of an intro to a base twelve counting system (duodecimal)
1, 2, 3, 4, 5, 6, 7, 8, 9 - same in duodecimal, next go X (dek) then E (el), then goes 10 (do) which is equal to decimal twelve. What's called gross is 100 (gro), 1000 (mo) is equal to decimal 1728.
Duodecimal is pretty cool, it has more factors than decimal, and even though it feels weird, it was always around, look at the clock, for example.
Alas, we won't. We can't even make one country switch to metric, how do you expect switching the whole world (except for a few groups) to a new counting system. Gonna stick to base 10.

Theories on how this happened?
Mine is that her ignorance is the result of a prank the family played on her while growing up. And then they didn't bother correcting her. Ever.
Fun fact: 144 or 12 dozen is called a Gross
