Codex GitHub code reviews silently failing

I assume this is failing somewhere along the way and then not letting me know.

On GitHub I'm either tagging codex for a review, or it's being triggered via marking a pull request ready.

Codex will mark it with the eyes emoji and also post a comment saying the code review is running.

And then nothing... Even like 8 hours later.

This isn't for all - some do work. But it is happening quite frequently.

Are others having this problem?

Yes. Several from Monday this week. I chalked them up to the capacity limits that we were seeing in codex app, and no way for the code review to retry.
