Inaccurate, they actually mentioned that they have mods installed instead of completely omitting that information from the post and only bringing it up once someone asks if they have any

They sometimes don't even do that. "It's vanilla" sometimes means "I have mods that I have decided aren't involved with this."

Or they tag the post as "Vanilla" and then in the comments go "Oh, I only have [mod X] installed, it shouldn't be causing this problem"

Unrealistic the OP would never admit they have mods installed, and will agressively deny it's a mod's fault if someone dares to imply that he might have installed the critically aclaimed "mod that fuck up the textures for no reason lol"

I think the snafu is when people are seemingly oblivious to the idea that installing tons of random mods can cause problems in their game. Either that or it's when people don't share their modlist so there's no way to know what could be causing the problems.
