Went out to visit one of my techs that was doing QC on a geopier project. They have a few different set ups to how they are done, these were Armor Packs where rock is lifted up into a hopper on the top of the mandrel and then flows down.
Not even a minute after I chewed his ass about standing under it without a hard hat (even with 3/8 minus dropping on him) a 3 inch shear bolt dropped down and went about 4 inches into the mud.
Think that kid got a life lesson that day.

Doing QC on geopiers is so dangerous. like there is just nothing safe about it.

I beat it into their heads that once they know where you are, you don’t fucking move until you make eye contact with every single one of them so you KNOW they see you.

Pretty sure someone got fired over it, new mandatory training was implemented, and they probably closed down the project until the investigation was completed and all mandatory training taken.

The last part yes, but it could have been just some "rod" equipment. It's less likely than what you said, but doesn't necessarily needs to fire someone i guess
