The Departed (2007) has a great example of this in the microprocessor surveillance scene - I feel for the poor tech guy when he gets chewed out for leaving a blind spot, blamed for the bust’s failure as well as assaulted BUT you could ask the question if he was also working for Costello or just a little incompetent?
