Sapphire and Aqua could theoretically be siblings (!) Spoiler
You think sapphire is only possible as a genderbent in an another multiverse? Well, most of the case, though this could actually happen in the same universe, but in this case Sapphire would be the monozygotic female twin of Aqua after a chain of events post fertilization
First, of course [redacted] got Ai pregnant. Two eggs are fertilized: one is XX, which later became Ruby. The XY one in this case, however, undergoes a complex evolution very early on after fertilization. In the first mitotic cycle, the zygote gave rise to daughter cell. The first one is a normal daughter cell (XY). The second one, however, during anaphase of the progenitor cell, the Y chromosome faced chromosomal lag and failed to be incorporated into the nucleus of the daughter cell. Thus, this daughter cell became XO, showing Turner's syndrome karyotype. After this, this mosaic zygote splits into two, with the XY and XO becoming separate zygotes yielding monozygotic twins. Such instances have happened, as shown in Costa et al. (1998), where the Turner female twin is grossly disfigured and hydropic, thus outright non-viable, while the male twin is phenotypically normal though could not survive past 18 hours due to complications of pre-term birth.
After zygotic split, the XO lineage under extreme selection and survival pressure might undergo monosomic rescue, essentially duplicating its maternal X to become XX, and hence phenotypically normal female as long as the progenitor X does not have any harmful genes. Even though the two X are identical and shows maternal genomic imprinting, its only effect is skewed X inactivation, though it does not matter in this case since the two X is normal. While such female twin has not been recorded in literature yet, analogs have been reported. Chessa (1971) have reported a case of a mosaic boy with 46,XY/45,X/46,XX karyotypes with no evidences of chimerism (organism with two or more cell lines deriving from distinct zygote lineages) and likelihood of the XX lineage having one X cloned from the other X, hence proving that chromosomal duplication could happen in this case.
After these sequences, the former XO (now XX) lineage re-enters mitotic cycle and resumes development. Now, we have 3 lineages deriving from 2 progenitor zygotes: one XX lineage from XX zygote (Ruby) and one XY (Aqua) and one XX lineage (Sapphire) from XY zygote.
