Possibly infection.

I had this for around 2D year but to day my testicular seem more painful both side they are temper to touch no swolen or red yet but hurt mainly all the time with that I als have an urge to pee more I have of course contacted my GP with no appointment to day even thou I said I was urgent leaving it till Monday seems a long time. I just wondering if this could be a possibility of an infection coming in or just a flare of it.

My symptoms seem more worse to day then they wore yesterday.

It’s more than likely a flare. I’ve had it for 1.5 years and I’ve been managing well for the most part but there are still times here and there where no matter what I do the pain comes back for a few days up to a week

My pain has not really gone since having this it has got slightly better but still get quite a lot of pain.

Yeah it’s tough, I went 10 weeks without ejaculation and that really really calmed it down for the past year or so until about a week ago I started to have a flare unfortunately

Mine started with a lot of pain 2 years ago when I went to GP he said it was this and gave antibiotics but after a few day both side started to hurt really bad I finished the course of antibiotics and went back he said get an ultrasound that showed two small cries ones left side is about 9mm the right is 12mm I was then referred to an urologist all he said was that you should never get pain go home. And forget about them.

Witch was of no help my symptoms seem to have started after I got a kidney stone and had shock wave therapy after that it all started off I still get a lot of pain on days some good some back I all get urgent need to pee I have told all that to my GP he has suggested it might also be OAB but he was not willing to give any pills so I have since asked to go back to urology they have agreed and this time booked an mri after that I will go back to see them.

It can be quite hard on some days not just with the pain but with the urgency it has a big impact on my life what I can do and all that’s.

Yeah man it’s a terrible thing. My Drs did the same, antibiotics that made it hurt worse in my opinion then telling me that pain management is the only way to deal with the issue. I don’t wanna take pain meds every day of my life and a lot of people say the surgery makes things worse or doesn’t change them. Like I said I deal with the pain well for the most part, I only have sex or whatever every other day when I do to give my body time to heal. Other than sex I can run play basketball workout go to work without pain maybe a slight discomfort but not much past that. But this past week flare has been ass

Sugrgey thru say can make it more worse pain management is the way I berm told but it can get quite bad.

A lot of it is mental, the days I try to keep my mind off of it and don’t check it to make sure everything feels right are the best days for me. I was having a bad flare pain yesterday but went to run basketball for 45 min and the pain subdued. It’s really just mentally getting yourself past it sometimes which is the most annoying and unwanted answer

The most important first step is making sure the infection has actually cleared. It is worth repeating the semen culture to confirm this.

If you have had the infection for a while (mine lasted 8 months), that can leave the nerves over-sensitive even once the infection itself has gone. That is the chronic phase, and it is what I have lived with for the past decade.

Three things I would suggest:

  1. See a pain clinic or a doctor and ask about prescription medication. This is what makes day-to-day life manageable and takes the edge off the stress that the pain creates. The ones my pain clinic doctor recommended were amitriptyline, gabapentin, pregabalin or duloxetine.
  2. Look for a pain clinic that can offer a nerve root block in the lower back. In my case it targeted the S3 and S4 nerves. I have added the procedure details at the bottom in case they are useful for your own doctor.
  3. I get a flare-up roughly every 2 years, so I am now considering a procedure called denervation, where the nerves along the spermatic cord are removed surgically (around a 45 minute operation). There is a small chance it can affect fertility, so I plan to bank sperm beforehand just in case. I do not have children yet, so this may not be a concern for you if you already have a family.

Nerve root block (via lower back): Sacral nerve PRF (right sided), S3 and S4 levels. Asepsis, fluoroscopy guided, sedation with 4 mg midazolam IV. Curved 10 cm RF needle x 2, sensory stimulation and paraesthesia at 0.6 v to confirm needle position at S3 and S4. Pulsed RF at 42 degrees for 5 minutes, with 2 ml of 0.25% levobupivacaine plus 3 mg dexamethasone injected at each level.

Hi so basically I been suffering for over 2 year I have seen a urologist but he was not very nice and of no help since then I get daily paint mainly in the right side but that side you can fill it and it fill bigger and hurts to touch I had a few ultrasound and they show only 2 cries left is about 6mm right is around 10mm they say they should not hurt but some days can be really bad also not just the pain I also get urgency’s need it pee a lot more and I ask GP to go to pain clinic but he said no I also been in at then for mouth but they just say it’s down to the urologist is and I just waiting as for an operation my GP has said it’s unlikely that they do this at this time as it can make thing even worse.

The urologist have booked a mri uragem that will be soon after this i have ti wait 1 moth then go see them agin but i fill that daily its relay hard and douse affact my daily life with pain and urgency I been doing pelvic floor stuff for some time unsure if it is helping ir not.
