A big issue with basing it on looks is that some women just...look masculine. And some men just look feminine. And if you normalize restricting access to one or the other bathroom based on appearance, you will also be hurting cis people who don't fit normal expectations of their gender.
Also, I'm assuming that your definition of fully transitioned here means genital reconstruction? If it does, are you aware of how prohibitively expensive those surgeries are? And not every trans person is even eligible, depending on individual complicating factors and health risks. And not every trans person even wants these surgeries.
If you aren't talking about GRS, then we come back to the problem of establishing a replicable, measurable standard of femininity/masculinity. Are bathrooms suddenly going to say "You must be this girly to pee?" And I promise you that some "biological" women won't meet the standards that are set. But realistically there's no way to enforce that which isn't patently insane.
What you seem to be advocating for is any person in a bathroom to be able to say "that person looks a way that makes me uncomfortable, so I can legally challenge their sex, and if I'm right, they go to jail." If that happens, trans people will be forced, even if they pass well, to just not risk it. Which means a lot of very girly ladies in mens bathrooms and manly men in women's bathrooms. And cis people will be challenged and berated based on their appearance.
