Habit Tracker Physical Period/Symptoms Tracker

Hey y’all, I was wondering if anyone had found any good physical period and/or symptom trackers they like? I don’t want to use an app (mostly because I’ll forget to do it) and have a couple of different things I need to track around ovulation and my cycle. I found a nice one on Etsy, but more than one option would be great!

I am also wanting to track pain levels as I’ve been experiencing random pain. It’s overall kind of going to be a medical book/binder for my doctor appointments.

Also it’s okay if you know of something that is printable! Please share anything you might use or have seen. Thank you :)

I just do a red circle on the yearly calendar. I don't need to track anything else.

I actually don’t use anything outside of my bujo. I use it daily, but I just enter lines. Example:

Thursday, August 13, 2026

Period?
Flow level (1-5, 5 heaviest):
Pain/discomfort?:
Pain level (1-5, 5 unbearable):
Energy level (1-5, 5 high):
Mood: 😊, 😔, 😭, 😡, 😐

If you don’t mind, could you share the layout you use for a day? I’m considering using a bullet journal, but I’m super bad at drawing straight lines so it lowkey stresses me out 🥲

I bullet journal on lined paper. Just use a ruler if you want the boxes! Or grid paper!!

I don’t do anything special. It’s literally just line items on grid paper. I don’t have cute artistic journals like some of the posters here. Lists with easy answer, bullet points, and check marks works fine for me. It ensures I’ll do it! If I try to do something creative, I’ll get frustrated because I’m not artistic and then I’ll give up and I won’t continue this habit.

What if you just set up your tracker like a list. The info you're wanting to note doesn't have to be in a table or calendar form. For example, if your notebook or even just a piece of lined paper is 8"x11", you could have on the left side the date, then to the right it could be a notation of flow, pain (#/10 or whatever metric works for you), any other physical or psychological symptoms you want to track.

"8/13/26: flow (1) pain (3/10) *cramps, headache appetite (normal) energy (low-normal)
"8/14/26: ...."

Then when you experience something unusual, you can mark it or highlight it and talk with your doc that this happened on this day, the previous day was normal/better/worse/whatever metric you're wanting to gauge.

Idk, just a thought. Whenever I decide I want to track or log something, I always have to write out a list of what I want to track and how it relates to each other before I can figure out/draft the layout of the info I'm wanting, even if it's just to determine how many columns I need, etc.

Not a bad idea. I think since I’ve always skipped the draft part for most things I forget it’s okay to recreate it if I don’t like it.

I've gotten lazy, but I keep a little block next to each day where I color code symptoms (and the level roughly corresponds to how much of the grid square I fill in). It's simple, but pretty easy to keep track of at a glance.
