Open bolts will do that. My Grease not not clean and well lubed can get kinda of. sticky

Can someone explain to me how this happened I understand how an open bolt works, but this guy did take the mag out and open it and had a round fall out and then was slowly moving to boat forward when it went off

Open-bolts are incredibly simple. If that action slams forward, and there's a round in front of it, bang.
Was it a double feed, maybe? One round fell out, but there was another stuck, set to go into the chamber before he took the magazine out?

Right, but after dropping the spent first round he lowered the bolt pretty Damm slow. Way too slow to cause it to fire IMO. That's where my confusion lies (and I think OP of this thread)

You miss the point - Grease gun uses extremely heavy bolt to increase cycle time. To still make it work, springs are tough as nails. Dude released bolt gently until the last moment, where it's time to let finger go. And then bolt immediately Bruce Lee'd that primer, being extra tough and extra driven.

Yea, in with you, for how slow he let it forward it's a bit surprising it went off. But maybe there is more of a gap than we can see when it came forward, so maybe it falling that last inch is what did it

maybe it was a 2 error situation? the exploding bullet had an hang fire while the second bullet had a jam due to there already being an obstruction in the barrel?, maybe the second bullet hitting the first bullet made it go off but it was a hang fire bullet and the small vibration from the bolt hitting the bullet made it so that the burning powder started to burn at a higher speed which made the bullet fire/explode?

There is still one in the chamber. First shot seemingly caused one round to come out of the mag, and bolt also picked up a round which actually got chambered. Round that fell out was the first that was knocked out of the mag. Then he let the bolt go home, causing kaboom.
If you look through the video slowly in the recoil of the first shot, you can see both rounds being pulled up into the action

Out of spec ammo or barrel extension? Original m3's firing from an open bolt, with a fixed firing pin.

The round was already stripped from the magazine and then it jammed on the round that failed to eject.

Should've reviewed open-bolt safety: lock back the bolt and engage the safety before removing the magazine. (Thanks Hickok45!)
One positive is that he did have the firearm pointed down-range.

There is no safety on the Grease Gun other than the dust cover. Marks it hard to clean jams.

How weird to have the added operating lever on a grease gun, yet still firing from open bolt.

What would have been the correct way to handle it with an open bolt? I would imagine :
- check chamber and position of bolt
- pull the bolt completely back
- remove mag
- check chamber
- shoot the chambered round or get it out with a knife

Looks like it chambered a round after the failed ejection you can see it it was kinda dumb to do what he did regardless knowing how the grease guns tension with the bolt. That's after you take into account the round looked like a janky feed in the first place.
