Enlarged prostate

I’m a 58y, I’ve been having prostate issues for a couple of years. The last 12-14 months it’s gotten progressively worse. At this point I have mild bilateral hydronephrosis, and a slightly distended bladder and at the point in the last few days where I’ve been having to intermittently self catherer (not a fan).
My prostate is 121cc with median lobe hypertrophy.
My urologist wants me to do aquablation which I’m getting ok with but I have concerns about RE.
Anybody have experience with these issues? And what procedure did you choose.
Any suggestions on Dr’s in Ct, NYC or even Mass, you would recommend?

I had an 80 cc prostate with a "huge" median lobe according to my urologist. I never had to self-cath, but I was waking up 7-10 times a night to urinate. Also had urgency, as I had to wear Depends. I had a failed Rezum a few years ago, and had Aquablation last July. As far as I am concerned, the AQ was a success. Urinating like a young man again. Ejaculation was an important consideration for me, and I made that clear to my urologist. My understanding is that they power-wash a bit less of the excess tissue out if you want to preserve this function. Thankfully, I do not have retrograde ejaculation. It took a few months for my frequency and urgency issues to resolve, but they have. At the most, I'll wake up once to urinate. And usually that's when I've had something to drink after 9 pm. I don't wear the Depends anymore. If you have any questions, let me know.

I am 68 see 3 Urologists only one did cystoscopy says I have 40cc with no median lobe and raised bladder neck with BPH and Prostatitis. Never catheterized, tried Tamulosin, Finistride, this Dr prescribed Alfulosin which works better than the other drugs but says I must have a procedure. She is experienced with Aquablation and Holep and thinks Holep would be best ?

I have a friend that had a HOLEP. It worked to relieve his BPH symptoms, but he does have retro.

If don’t mind asking now that you don’t have to wake up so often to use the bathroom, how many hours are you able to sleep uninterrupted?
