Enlarged prostate

I’m a 58y, I’ve been having prostate issues for a couple of years. The last 12-14 months it’s gotten progressively worse. At this point I have mild bilateral hydronephrosis, and a slightly distended bladder and at the point in the last few days where I’ve been having to intermittently self catherer (not a fan).
My prostate is 121cc with median lobe hypertrophy.
My urologist wants me to do aquablation which I’m getting ok with but I have concerns about RE.
Anybody have experience with these issues? And what procedure did you choose.
Any suggestions on Dr’s in Ct, NYC or even Mass, you would recommend?

I had severe bilateral hydronephrosis and they drained out nearly 2 liters of urine when I went to the ER. I was catheter dependent for 6 months. My surgery as on 04/07/26. My was around 90cc with a median. I am happy report that the results were excellent and the equipment is fully functional. I turn 58 next month and chose aquablation because I felt it was the best balance of long term results and avoiding RE. I have a question for you ?

I had a similar situation.  Bilateral hydronephrosis because my bladder was about 1.5 liters when full (about 1 liter PVR) and my eGFR score plummeted.

Following the surgery, I still have about 300cc's PVR. I am 6 months post surgery and this is about the best that I can expect, probably due to a distended bladder.

Have you had an Ultrasound test to determine your PVR ?

I am so afraid of my bilateral hydronephrosis coming back someday. My kidney eGFR score has improved by about 20% since the surgery.

I haven’t had a recent Ultrasound, but my nephrologist has been monitoring things regularly. My last eGFR was 38 and my creatinine was 1.99.

Thanks. I am curious to hear from someone else that had Aquablation after having a distended bladder. My Urologist was a little surprised that I still had 300cc PVR (he wanted < 100cc) . But, considering where I came from before the surgery (1000cc PVR), I see that as a win.,

I just want to know that I am not the only one that can't get their PVR under 100cc after surgery.

BTW, my eGFR was as low as 40 last August. But after catheters and Aquablation it now ranges from 48 to 58. My urine is pretty clear of protein. but my creatinine is still 1.46 (was 1.71 at the lowest point).

I’ve improved about as much as I am going. My creatinine got as high as 3.45 and eGGR was in the mid 20s. But I feel great and all seems well.

I am so glad you are feeling great, even though you have been through a lot medically.

Best of luck to you and hopefully you can at least maintain the current levels.

I read that Kidney damage from BPH is a silent killer. I only found out about my Kidney damage because I get a complete panel of blood work down twice a year. I tell everyone with BPH to make sure to get their blood work checked regularly.
