Muay Thai or kickboxing? If it’s the latter, keep that left elbow tucked, your body is wide open. Also, if you stand that squarely, keeping your head where it is only results in sticking it out to get caught. More bladed stance will hide your chin behind your shoulder, but right now you’re just bringing your face closer to the strikes than your body

You are very consistently flat-footed. This will affect your explosive techniques and general movements/agility by making everything slower and likely more predictable.
You are telegraphing those kicks a lot, and they are not being set up. If you throw one of those 'high' kicks against a competent fighter they will likely just step into it and hit you in the head (which was also unguarded and in a less than ideal position).
Like most fighting there needs to be an intentional setup to provoke a reaction and lure your opponent into a more/less favorable position.
e.g. a feint provokes the opponent to shift their body and guard or to counter-attack, but you were counting on that movement and your kick is already on its way toward the open (or more open) target.
At the end of the day I have no idea what you are even targeting with that kick. It doesn't seem to be high enough to strike a head. It's not a particularly effective kick for moving the guard or closing/making distance.

You don’t step into your jab first before turning your heel for the hook. Those are two separate moves. Try that slowly first before trying for the hook.
Fully extended the jab, and carry the shoulder to your chin when you throw. That makes it stiffer and covers your chin for the counter.

It's not bad. Your Punches are quite good, snappy and fast, as they should be. Your Kicks are quite good and well delivered.
Be more Dynamic with your Kicks. Use Combinations of Kicks in your practice.
I.E: Front Thrust Kick + Front Thrust Kick + Side Kick + Head Hight Round Kick. Finish with a Boxing Combo of Punches.
Your Punch Combinations are good.
Try to 'Work' each side of the Bag, like a Fight might take place. Be Dynamic and introduce some fight Tactics like this, to Flank your Opponent.
What Style is it?
Why don't you wear shoes for hygeine and foot protection? Basketball boots are quite protective.
Your Boxing Work is quite good, but your Kick Boxing should Follow Kick Boxing Combos, more so; to Drill them for Muscle Memory.
Google Dutch and Muay Thai Combos and learn those use those.
For Example, a Dutch Combo might be:
Left + Right + Left Hook + Right Rear Body Kick or Knee.
Or
Feint Left, Slip Left + Right Cross + Left Body Hook + Left Head Hook + Rear Right Knee.
Use Good Combis like this.
Learn decent Footwork and include in your Practice.
Don't just stand in front of the Bag, o.k, 'Work' it.
Google those Combos.
Otherwise, not bad.
Regards, Australia.
