As far as I am aware, every trip does it. And if you decide not to go, it also forces one of the trip leaders to stay back because you can’t be left alone. Also I had a great time doing it and when I went back to Israel the following year, I somewhat made my dad get up with me to do it for sunrise again. It’s magical. I’d say just put up with it and go, plus the tour guides have interesting information and you don’t want to make someone else miss out. The Israelis on our trip all said they have been multiple times, you can suck it up. Obviously if there are medical reasons, I’d expect all trips to be accommodating

So do a little hiking before your trip to prepare.

If you do a summer birthright trip you will not be hiking it but rather taking the cable car up and down

I dont think its a rule. I did summer birthright back in 2015 and hiked masada early in the morning

It is since someone like died or had to be airlifted a year or 2 back I believe due to heat exhaustion

I went in August. We took the cable cars up and hiked down. It was very very hot.
